Application Forms
All new and returning families must complete the ECE Health and Permission Form or the Grades 1-12 Health and Permission Form by the first day of school. All new students, incoming Kindergarteners and incoming 7th graders must submit new immunization records by the first day of school. All forms can be returned to the Front Office or emailed to erica.stoltz@mhacyos.org.
All interested families must complete the MHA Application Form for each student. Records, transcripts, and standardized test scores from all previous schools attended must be submitted to the MHA Office or erica.stoltz@mhacyos.org for an application to be considered complete. An application fee of $110 per child must accompany the application (max $220 per family). This fee is non-refundable. Please pay online at mhafyos.org/donate or send a check to the MHA Office.
Students applying for Grades 9-12 may be required to take a Math and English Placement Exam. We also request copies of any private or county testing results. Transcripts are required before a student can be accepted. We strongly urge families to make a school visit to tour and sit in on classes, or a zoom interview with administrators if applying from out of town. Dorm students must submit a Medical Authorization Form and a copy of their insurance card.
Administrators will review applications and make a decision when all paperwork has been received, and families and students will receive notice of acceptance by email.
